How long does GLP-1 last?

One of the most important differences between natural GLP-1 and synthetic GLP-1 (pharmaceutical GLP-1 receptor agonists) is how long they remain active in the body. Natural GLP-1 is extremely short-lived. Once it is released in response to food, it is rapidly broken down and cleared from the bloodstream relatively quickly, giving it a half-life of roughly 1 to 2 minutes.

Synthetic GLP-1, on the other hand, is engineered to behave differently, designed specifically to resist the processes that normally break down natural GLP-1. As a result, synthetic GLP-1 can remain active in the body for vastly longer periods of time. Depending on the particular drug, pharmaceutical GLP-1 can have half-lives up to 7 days. Newer formulations are being investigated with the goal of extending this duration even further, potentially to 1 month.

One of the goals of the pharmaceutical industry in extending the half-life of synthetic GLP-1 is what is commonly referred to as dosing convenience—the ability to administer a medication less frequently while maintaining its effects. But it is important to understand what this means for the patient as it relates to synthetic GLP-1.

The GLP-1 receptors in the body are naturally activated in short bursts—a few minutes at a time. By contrast, pharmaceutical GLP-1 can keep these same receptors activated continuously for days or weeks at a time. If once-monthly pharmaceutical GLP-1 is approved for patient use, receptors throughout the body, including in the gut and brain stem, may remain activated beyond their natural course by a factor of 20,000.
